It is quite easy to find out the originating domain for a flash ad:
Step 1: Right click on the advertisement and select "Settings".
Step 2: Click on the second button from the left - it looks like an eye on a monitor - in the little popup that appears.
Step 3: It will say something like, "Allow s.ytimg.com to access your camera and microphone?" Write down the URL mentioned (in my case, s.ytimg.com) and block it.
